Physical Requirement for Marching Band

Who:

All students are required to participate in Marching Band and must have a physical completed. Physicals need to be completed every year by a physician.

What:

Physicals: must be completed by a physician, and they must fill out the EL2 form included in the physical packet linked on this page. Students/Parents must also provide proof of health insurance

When:

Forms must be uploaded in time for students to be cleared to participate by the start of Band Camp in July.

Where:

All students must be registered through https://athleticclearance.fhsaahome.org. Once an account is created and the student is registered, the forms are to be uploaded there.
