Who:
All students are required to participate in Marching Band and must have a physical completed. Physicals need to be completed every year by a physician.
What:
Physicals: must be completed by a physician, and they must fill out the EL2 form included in the physical packet linked on this page. Students/Parents must also provide proof of health insurance
When:
Forms must be uploaded in time for students to be cleared to participate by the start of Band Camp in July.
Where:
All students must be registered through https://athleticclearance.fhsaahome.org. Once an account is created and the student is registered, the forms are to be uploaded there.
A Note Regarding Health Insurance

What is Kid Guard?
Kid Guard is a cost effective way to get your student covered with enough medical insurance to participate in athletic activities (Ex: Marching Band). The School Time Accident plan ($15!) or the 24 Hour Accident Insurance Plan ($65) are both cost effective options you can use to insure your child to participate in Marching Band.
